The death benefit will be paid in a lump sum to the named beneficiary (or beneficiaries) of your choice. You designate your beneficiary using the Retiree Health Plan Beneficiary Benefit form. If you do not designate a beneficiary, or if the designated predeceases you, the Plan will pay the benefit according to its rules.
