6.2 Pharmacy Chains

Participants and eligible dependents enrolled in Kaiser Permanente HMO must obtain their prescription drugs from a Kaiser Permanente pharmacy. Participants and eligible dependents enrolled in Blue Shield PPO Plan or UnitedHealthcare HMO may obtain their prescription drugs through contracted pharmacies. Participants and eligible dependents may review contracted pharmacies through the provider websites at: www.myuhc.com for UnitedHealthcare or www.anthem.com for Blue Shield.

Below is a sample of major retail California pharmacy chains available to participants and eligible dependents enrolled in Blue Shield PPO Plan or UnitedHealthcare HMO, as of the date of the printing of this Summary Plan Description:
