AMENDMENT NO. 4
TO THE SUMMARY PLAN DESCRIPTION OF THE SOUTHERN CALIFORNIA IBEW-NECA HEALTH TRUST FUND ACTIVE HEALTH PLAN
(RESTATED AS OF JULY 1, 2022)

This Amendment to the Southern California IBEW-NECA Health Trust Fund, Active Health Plan Summary Plan Description (“SPD”) is made by the Board of Trustees of the Southern California IBEW-NECA Health Trust Fund (“Board of Trustees”) with reference to the following facts and circumstances:

1. The Board of Trustees wishes to amend the SPD to reflect the changes in the benefits available to the Material Handlers Agreement effective for work month February 2024 (June 2024 eligibility month) as outlined below.
2. The Board of Trustees has reserved to themselves the ability to amend the SPD from time to time.
   
   

   NOW THEREFORE, effective February 1, 2024, the SPD is amended as follows:

   1. Article 2, Plan Benefits Available to You, Section 2.4, Material Handlers Agreement is replaced and restated as follows commencing with February 1, 2024 work month (June 2024 eligibility month):

      • Hospital/Medical/Prescription Drug Benefit
        • Alternate Kaiser Permanente HMO Plan Only
      • Health Reimbursement Arrangement
      • Advocacy and Assistance Benefit
        • MedExpert
      • Member Assistance Program (MAP)
      • Life Insurance/AD&D Benefits
        • Anthem Blue Cross Life and Health Insurance Company

All other terms and conditions of the Summary Plan Description and Plan, shall remain in full force and effect.
