Amendment 2 - Anthem Blue Cross shall replace Metropolitan Life as the Plan's Life Insurance and AD&D benefit provider. References in the SPD appearing in the Table of Contents on page iii and on pages 4, 29, 57, 58, 63 and 64 and elsewhere in the SPD to Metropolitan Life shall be changed to Anthem Blue Cross wherever those terms appear.
