United Concordia Dental Plan/Concordia Advantage Plus
United Concordia Dental Plan will replace the current PPO provider, Delta Dental
effective January 1, 2011. All participants currently enrolled in the Delta Dental PPO
Plan will automatically be enrolled in the United Concordia Dental Plan.
The United Concordia Dental Plan offers coverage at affordable rates and access to
a large dental network of 21,000 practicing locations in California. With this plan you
may visit any licensed dentist; however, you can maximize your benefits by visiting a
Concordia Advantage Plus network dentist. You can search for a network dentist using
Find a Dentist on UnitedConcordia.com. Just select the Concordia Advantage Plus
network and your search preferences.
You will be receiving your ID card(s) later this month from United Concordia Dental and
can begin using them on January 1, 2011 (one card for a single subscriber and two for
a subscriber plus dependents). If you are visiting your dentist before the end of the year,
be sure to tell him/her that your dental plan is changing effective January 1, 2011.
You may call United Concordia Customer Service at 1-800-332-0366 24/7 and use their
Interactive Voice Response System if you have general questions, to find a network
dentist, hear coverage details and more or to speak to a live representative Monday-
Friday, 5 a.m.-5 p.m. PT.
Beginning January 1, 2011, when the United Concordia Dental Plan is "live" or contains
specific data for participants and dependents of the Southern California IBEW-NECA
Health Plan you may access benefits information, such as claim status, procedure
history and more, using United Concordia's online member tool, My Dental Benefits, in
the Members section of UnitedConcordia.com.
The Trust Funds' Administrative Office is working closely with United Concordia Dental
to ensure that the transition to the new plan is as seamless as possible. Please feel free
to contact the Trust Funds Administrative Office at the contact numbers provided on the
letterhead if you have questions or concerns regarding the transition from Delta Dental
PPO to the United Concordia Dental Plan and the Concordia Advantage Plus network.
