The Southern California IBEW-NECA Health Plan may use the limited quantity of private health information available, that is, information that constitutes protected health information as defined in the Privacy Rule of the Administrative Simplification provision of the Health Insurance Portability and Accountability Act of 1996 ("HIPAA"), for purposes of making or obtaining payment for your care and conducting health care operations. Health Plan has established a policy to guard against unnecessary disclosure of your health information. The Trust Fund's HIPAA security practices encompass administrative, physical and technical safeguards.

You may request a copy of the Southern California IBEW-NECA Trust Fund's Notice of Health Plan's Privacy Practices in writing. Please note, this Notice is in the process of being revised. You will receive the updated Notice once the revision process is completed (5/21/2013).
