It is important that you respond to Consova regarding the eligibility of your enrolled dependents so the health coverage for your dependent(s) may continue without interruption. Please contact the Consova Dependent Eligibility Verification Assistance Center at (855) 864-9391, Monday – Friday 7am to 3pm PST, if you have questions or need assistance.
The link here is a sample of Consova's Reminder Notice being mailed at your address on record with the Administrative Trust Fund Office, requesting that you confirm your dependent eligibility information and to submit the required documentation. This is not a passive verification process. Consova is focused on making this necessary process as easy as possible for you. A response is required. All information you provide to Consova is kept secure and confidential.
