For the Indemnity Plan, you will generally receive a higher benefit if you use a participating provider. For the two HMO plans, you must use an HMO provider except in medical emergencies (as defined by the HMO), or when you are travelling outside of the area served by the HMO.
HealthAdvocate has been engaged to help you find physicians, medical specialists and other providers and facilitate referrals for covered services within the Plan Networks.
Chiropractic Coverage: Kaiser Permanente uses American Specialty Health Network for chiropractic. PacifiCare uses ACN Group, Inc. for the chiropractic benefit.
