Complete & Print Forms - You can complete most of the forms listed below right on your computer before you print. Simply click on a field in the form and type in the appropriate information. Then print the completed form, sign and mail it to the Trust Funds Office.

General

Pension Payment Options - This form provides you with an explanation of the various pension benefit payment options available to you under the rules of the Pension Plan.

Defined Benefit Beneficiary Designation Form - Use this form to designate the beneficiary(ies).

Direct Deposit Form (EFT) - Use this form to request pension checks to be deposited directly to your checkings or savings account.

Change of Address - Use this form to change your address. Be sure to fill it out completely and return it to the Trust Office. (English Version)

Forma Para Solicitar Cambio De Dirección (En Español) - Use esta forma para hacer un cambio de dirección postal.  Asegúrese de completer todos los espacios en la forma, y de enviar la misma a la oficina del Trust Funds.

Pension Plan Application Checklist - Use this checklist to help you complete an application for Age/Disability Retirement Benefit.

Defined Benefit Pension Application Form - Use this form to apply for Defined Benefit Pension. Be sure to fill it out in its entirety before returning it to the Trust Fund Office.

Application for Pre-Retirement Death Benefit Form - Use this form to apply for Pre-Retirement Death Benefits.

Employer Certification Form - For Pension Applications, please answer the following questions and return the form to the Trust Fund Office. In order to expedite the processing of your pension application, please have your employer complete the "Employer Certification" below, prior to returning to this office.

Authorization for NEBF to release Social Security earning information form - Use this form to authorize the National Electrical Benefit Fund (NEBF) to release to the aforementioned Pension Plan any information they may, at any time, request which the NEBF has in its possession relating to you.

Request for Social Security Release information form - This form may be used to request social security earnings information from detailed earnings information to certified total earnings for each year.

Return to Work

Intent to work after retirement effective date form - This is the current notice of rules concerning 1) starting benefits for early retirement and 2) suspension of benefits for early retirees returning to work in the Electrical Construction Contraction Industry. The notice lists the jobs and types of maintenance work that Early Retirees can perform and still continue to draw pension benefits.

Request for work after Retirement determination form - Use this form to request work after a retirement determination. To determine if the work you are, or will be performing, is permissible under the plan, please have the employer provide the information requested in this form.

Summary Annual Report - Pension Trust Fund - This describes the status of the fund at the end of Plan Year 2007.

Summary Annual Report - Pension Trust Fund - This describes the status of the fund at the end of Plan Year 2006.

Summary Annual Report - Pension Trust Fund - This describes the status of the fund at the end of Plan Year 2005.

Annual Funding Notice - This notice, which federal law requires all multiemployer plans to send annually, includes important information about the funding level of the Southern California IBEW-NECA Pension Plan, Plan Number 001 and EIN 95-6392774 (Plan). This notice also includes information about rules governing insolvent plans and benefit payments guaranteed by the Pension Benefit Guaranty Corporation (PBGC), a federal agency. This notice is for the plan year beginning 7/1/2006 and ending 6/30/2007 (Plan Year).
