Form W-4P (2012) - Use the Withholding Certificate to elect the rate of withholding for Federal tax purposes from your Pension benefit payments. Participants only need to file the form once, unless changes are needed, such as altering marital status.

Form DE-4P (2012) - Use the Withholding Certificate to elect the rate of withholding for California State Income Tax purposes from your Pension benefit payments. In compliance with Federal law, California Income Tax is not withheld from retirement allowance if the participant resides outside of California. Participants only need to file the form once, unless changes are needed, such as altering marital status.

In order to expedite the processing of your pension application, please have your employer complete the "Employer Certification" below, prior to returning to this office.

Authorization for NEBF to release Social Security earning information form - Use this form to authorize the National Electrical Benefit Fund (NEBF) to release to the aforementioned Pension Plan any information they may, at any time, request which the NEBF has in its possession relating to you.

Request for Social Security Release information form - This form may be used to request social security earnings information from detailed earnings information to certified total earnings for each year.

Intent to work after retirement effective date form - This is the current notice of rules concerning 1) starting benefits for early retirement and 2) suspension of benefits for early retirees returning to work in the Electrical Construction Contraction Industry. The notice lists the jobs and types of maintenance work that Early Retirees can perform and still continue to draw pension benefits.

Request for work after Retirement determination form - Use this form to request work after a retirement determination. To determine if the work you are, or will be performing, is permissible under the plan, please have the employer provide the information requested in this form.

Summary Annual Report - Pension Trust Fund - The Pension Protection Act eliminates the Summary Annual Report that has, until now, been provided to you annually, and requires all defined benefit pension plans, funded and underfunded, single and multiemployer plans to distribute annual plan funding notices to all plan participants and beneficiaries, labor organizations representing participants, employers having an obligation to contribute under the plan, and the PBGC. The Annual Funding Notice was mailed to all Plan participants on October 28, 2010 and is available on the Plan website at www.scibew-neca.org.
