For Retirees UNDER Age 70.5

If you are a Retiree UNDER Age 70.5, you will need to take the following steps:

1. Complete, sign, and date the Annual Pensioner Verification Form
   Please contact the Fund Office to obtain a replacement copy of your APV form
   
   
2. Please contact the Administrative Trust Funds Office to obtain the Request for Social Security Earnings Information Form
   
   
3. Return the completed form(s) to:
   SOUTHERN CALIFORNIA IBEW – NECA TRUST FUNDS
   P.O. Box 6652
   Pasadena, CA 91109
   
   
4. And that's it!
