Important Notice to Participants

Dear Participant:

The collective bargaining parties have amended the Collective Bargaining Agreement and in response thereto the Trustees are amending the Summary Plan Description to reflect the termination of Domestic Partner coverage effective September 30, 2016.

The Trustees are extending coverage for existing Domestic Partners through December 31, 2016. No new Domestic Partners may be added on or after October 1, 2016 and there will be no coverage for claims incurred on and after January 1, 2017 for Domestic Partners.

Additionally, references within the Summary Plan Description (SPD) appearing on pages 13, 14 and 15 and elsewhere in the SPD to Domestic Partners are deleted.

This Notice is a Summary of Material Modifications ("SMM") within the meaning of Section 104 of the Employee Retirement Income Security Act of 1974. An SMM describes changes to the information provided in the most recent SPD. The SMM describes important changes to the Plan effective as of the date listed above. Please keep this SMM with your SPD for future reference. Please contact the Fund Office if you would like to request a copy of the SPD or any SMM relating to the Plan.
